To print the form, wait for it to load and then press the print button in the Acrobat Reader tool bar. Print the form, fill it in, and send it to the address indicated. To save it to your hard drive first, right-click the button below and choose "Save Target As". Save it to a folder you'll remember, then double-click it to open it.
